I feel that there is no shame in our bodies - and the first step towards a healthy sex life is to love your own body, and be comfortable with your entire body. Find a partner who you can trust and understands this, and explore each others bodies - if you are shy about having your partner touch part of your body, for example, the anus, then practice touching there, in a safe and loving setting, using massage and softness to help each other accept the body in its entirety.
Second step - open mind. Dont judge what you like or dislike - what turns you on or off. Just learn it about yourself. If youre doing your yoga and start feeling unhappy about the way you look, simply notice the feeling, take a nice, slow breath in and go back to paying attention to the stretch and to your breathing.
How to Get the Most out of the Following Routine
•Notice how your body relaxes as you breathe out, which will allow you to stretch a little farther.
•Do not stretch so much that you feel burning or pain.
•If you feel like your body has hit a concrete wall, then youre at your limit. When you get to that point, simply breathe and feel your body relaxing for a couple more breaths, then let the posture go.
•Relax completely in between repetitions, feeling your muscles letting go of the stretch, feeling them relaxing. The full routine will take about an hour.
The vast majority of men and women are, through sexual relations, losing the very thing he or she needs most to find real, lasting satisfaction.
There is a vital life-current activating force or essence in the vital fluids that can be of tremendous benefit to the glands and nervous system if these fluids are conserved and reabsorbed through the process of Tantra and transmutation. Conservation makes it much easier to be content with very little and results in much greater sensitivity to those things that bring joy, inspiration and happiness. The breathing excersises and meditation practices that I will be writing about require sexual energy to be effective and benefical, and at the same time, is needed for transmutaion. Successful control of the senses requires strict control of the mind gained through long periods of concentration in the third eye and inner sounds through meditation.
Depending on how one uses this information, one can free oneself more and more from the need of physical sex to find a superior and long lasting fulfillment through conservation and transmutation. Why go for the temporary excitement of sensual pleasure (loss of vital fluids) if it is possible to enjoy long periods of bliss, joy and experiences of profound spiritual inspiration, oneness or wholeness? Perfect, uninterrupted celibacy, if skillfully managed, can also mean a lifetime of uninterrupted joy, bliss, vitality, virility, better health, inner strength, greater memory and wondrous mental powers enchantment inevitably leading to some very high states of conscious realization. In this lifestyle, temporary thrills are sacrificed for constant joy.
Yoga Meditation Breathing for Better Sexuality:
This is a basic meditation that will help your ability to simply sit and notice feelings without doing anything about them. With practice, you will be more able to sustain your sexual energy during love making and will be more able to build your sexual desire during a session. As a result, you will have more rich experiences - and stronger orgasms.
Where When to Do This:
Any time, anywhere you have a moment.
Best place is in a quiet spot where youll be undisturbed for 5 - 20 minutes.
Best time is in the morning - but any time is good.
The Meditation Techniques
•Sitting comfortably or lying down.
•Be in a position where your body will feel least inclined to move.
•If youre lying down, have your arms by your sides with a little space between your body and your arms.
•If youre sitting, your arms can be resting loosely in your lap, elbows slighltly open.
•Have your face chin pointing forward, so that your air passages are fully open.
•Eyes are open and relaxed, not looking at anything in particular.
Once you are settled into place, all you need to do is pay attention to your breath as it comes and goes.
As you breathe in, hear the air rushing into your head, feel your body opening up. As you breathe out, follow the breath out into the room. Do this for however long you have. 3 - 5 minutes is a good start. If you become comfortable with 3 - 5 minutes, increase the duration 1 minute at a time.
During this meditation, its very common for people to have feelings and thoughts come up.
You might notice a flood of emotions start to rise up, or your mind might start to race. Many people start to feel slightly irritated with the exercise, or they start to feel .
This is OK and is a sign that the meditation is going normally and is working. It shows that your mind is starting to relax, and as it is relaxing you are starting to let go of thoughts and feelings that you have been keeping a lid on. The remedy for this is to simply notice the thoughts and feelings and let them go by breathing in and out. Draw a slow breath in, noticing the air flow in, noticing how your body opens up - and youll refocus and be back into the meditation. If you can continue, you will eventually find that these feelings subside and you reach a deeper state of meditation. The benefit to your life is that it makes you very patient and trains you to enjoy the moment.
With Your Partner:
There are some wonderful variations on this meditation if you want to do it with your partner. With any of these, you may sometimes find your breathing ends up being synchronized. If you are aware of your energy bodies, you might also notice that these exercises start synchronizing your energy bodies, so that the rhythms, flow and colours begin to be in harmony with each other. When that happens, its very beautiful.
Here are three variations of this meditation that are lovely to do with your partner:
1. Simply practice together, in the same room, each of you paying attention to your own breathing.
2. Sitting, facing each other, do the meditation by paying attention to your partners breathing. You may find it hard to keep your attention on your partners breath. If your mind wanders, thats OK, just bring your attention back to the exercise.
3. Both of you lie down, facing the same direction, one partner snuggled up behind the other. Decide ahead of time how long you will do the meditation and if it is or is not allowed to go farther into sexual exploration. Decide ahead of time if you will be paying attention to your own breathing or to your partners, or if youre going to start with yourself and later shift to noticing your partners breath. Then simply notice the breath as it comes and goes, the movement of your bodies, the sounds, the feel of the breath on your skin.

[0001] The present invention relates to a process for the sterilization of steroids, particularly glucocorticosteroids and to the use of steroids so sterilized in sterile pharmaceutical formulations.

[0002] Sterilization is a process performed to ensure that there is complete freedom from microbial contamination. Sterilization is especially done for pharmaceutical formulations which are to be directly introduced into the body and its cavities. Such formulations explicitly include ophthalmic preparations, nasal preparations, ocular preparations, injectables, transdermal patches, depot preparations and the like. Such sterilized preparations involve two main methods of preparation. First route is that the active ingredient is sterilized and the formulation is prepared aseptically or the final is prepared, packed in the desired container and then sterilized. The second route is known as a terminal sterilization technique.

[0003] Certain formulations such as respules or aqueous nasal preparations, ophthalmic preparations and the like that involve steroids as the active ingredient are usually prepared by the first method described above. The most important reason being that these formulations are usually packed in containers made from LDPE which are not suitable for terminal sterilization.

[0004] Several patents disclose the methods of sterilization of active ingredients. PT-A-69652 discloses the cold sterilization of micronized glucocorticosteroids using mixtures of ethylene oxide and carbon dioxide, since according to PT-A-69652, steroids in powder form are not stable at temperatures above 60.degree. C. Specific examples of glucocorticosteroids are prednacindone, budesonide, dexamethasone and prednisolone, and salts, esters and fluro derivatives thereof, including dexamethasone acetate, dexamethasone phosphate, prednisolone pivalate and 9-alphafluoro prednisolone.

[0005] However, ethylene oxide is toxic and when it is used to sterilize glucocorticosteroids it has been found that the residual amounts of the ethylene oxide contravene pharmaceutical guidelines, which require very low levels of residual ethylene oxide. Accordingly this method has been found to be unsuitable for producing therapeutically acceptable glucocorticosteriods and formulations thereof.

No. 3,962,430 discloses a method for the production of sterile isotonic solutions of medicinal agents, which comprises adding the agent to a saturated solution of sodium chloride in water at 100.degree. C. and then heating the mixture at 100-130.degree. C. This method is not suitable for suspensions of fine particles of steroids, which are intended for inhalation because the water, and the heating and cooling involved, produce unfavourable changes in the size of particles. However at the temperatures of these processes glucocorticosteroids suffer significant degradation and are subject to changes in their surface structure.

Sci., Ed. 2, 1974, pp. 167-174 recommend the use of irradiation to sterilize glucocorticosteroids. However when such irradiation is used to sterilize certain finely divided, e.g., micronized steroids such as glucocorticosteroids, they are significantly degraded.

[0009] WO-A-96/09814 to Andaris Ltd. relates to spray-dried particles of a water-soluble material with a mass median particle size of 1 to 10 .mu.m. The aim of the invention is to produce forms and respirable particles for use in dry powder inhalers. The water-soluble material is preferably a human protein or a fragment thereof, in natural or recombinant form, e.g., human serum (HAS), alpha-1 antitrypsin or alcohol dehydrogenase. Also combinations of an active material with a carrier were produced e.g., budesonide and lactose. It is stated generally that the microparticles produced can be sterile without teaching how this could or would be neither achieved or showing any proof thereof.

Attempts at terminal sterilization of pharmaceutical formulations, especially suspensions, e.g., aqueous suspensions, of glucocorticosteroids have all proved unsatisfactory. Such suspensions can not normally be sterilized by sterile filtration as most of the particles of glucocorticosteroids will be retained on the filter. Moist heat sterilization, e.g., steam treatment of glass vials containing the product, leads to an unacceptable change in particle size.

[0012] Various aqueous suspensions of finely divided glucocorticosteroids are known e.g., the budesonide-containing product known as Pulmicort.RTM. nebulising suspension. C. However, we have found that by this method final formulations show a considerable rise in impurity levels.

Alachua County Sheriffs deputies arrested a 26-year old Gainesville man earlier this week on charges of arson, destroying evidence, burglary and larceny, according to Gainesville.com. Police said Devin Anthony Perry broke into a Department of Corrections Probation and Parole office early Sunday and stole a small refrigerator containing urine samples from probationers. One of those probationers was Perry, police said, who had if a sample last Friday that tested positive for drugs.
Sheriffs Office speaker Art Forgey said that after Perry realized his sample had tested positive, he went back to the office at 0-dark-30 Sunday, shot out a window, climbed interior and separate the small, locked refrigerator containing the samples. After he climbed out, Forgey said, Perry gathered up the shards of glass through his blood without interruption them and then set a violence in some shrubbery beneath the window.
When probation officers arrived at the office in a circle dawn and speckled the smoldering shrubbery, they called police, who quickly determined which probationers might have had reason to want their samples to go missing and began investigating. Forgey said they found the sanguinary shards of glass in Perrys trash, wrapped inside some clothing.
Perry is session in the Alachua County Jail, but the refrigerator was still missing as of last Monday.

BETA-RECEPTOR AGONISTS
These drugs, which are bronchodilators, are the best choice for relieving sudden attacks of asthma and for preventing attacks from being triggered by exercise. Some agonists, such as albuterol, act mainly in lung cells and have little effect on other organs, such as the heart. These drugs generally start acting within minutes, but their effects last only four to six hours. Longer-acting brochodilators have been developed. They may last up to 12 hours. Bronchodilators may be taken in pill or liquid form, but normally are used as inhalers, which go directly to the lungs and result in fewer side effects.
STEROIDS
These drugs, which resemble natural body hormones, block inflammation and are extremely effective in relieving symptoms of asthma. When steroids are taken by inhalation for a long period, asthma attacks become less frequent as the airways become less sensitive to allergens. This is the strongest medicine for asthma, and can control even severe cases over the long term and maintain good lung function. Steroids can cause numerous side-effects, however, including bleeding from the stomach, loss of calcium from bones, cataracts in the eye, and a diabetes-like state. Patients using steroids for lengthy periods may also have problems with wound healing, may gain weight, and may suffer mental problems. In children, growth may be slowed. Besides being inhaled, steroids may be taken by mouth or injected, to rapidly control severe asthma.
LEUKOTRIENE MODIFIERS
Leukotriene modifiers (montelukast and zafirlukast) are a new type of drug that can be used in place of steroids, for older children or adults who have a mild degree of asthma that persists. They work by counteracting leukotrienes, which are substances released by white blood cells in the lung that cause the air passages to constrict and promote mucus secretion. Leukotriene modifiers also fight off some forms of rhinitis, an added bonus for people with asthma. However, they are not proven effective in fighting seasonal allergies.
OTHER DRUGS
Cromolyn and nedocromil are anti-inflammatory drugs that are often used as initial treatment to prevent asthmatic attacks over the long term in children. They can also prevent attacks when given before exercise or when exposure to an allergen cannot be avoided. These are safe drugs but are expensive, and must be taken regularly even if there are no symptoms. Anti-cholinergic drugs, such as atropine, are useful in controlling severe attacks when added to an inhaled beta-receptor agonist. They help widen the airways and suppress mucus production.
If a patients asthma is caused by an allergen that cannot be avoided and it has been difficult to control symptoms by drugs, immunotherapy may be worth trying. Typically, increasing amounts of the allergen are injected over a period of three to five years, so that the body can build up an effective immune response. There is a risk that this treatment may itself cause the airways to become narrowed and bring on an asthmatic attack. Not all experts are enthusiastic about immunotherapy, although some studies have shown that it reduces asthmatic symptoms caused by exposure to house-dust mites, ragweed pollen, and cat dander.
Managing asthmatic attacks
A severe asthma attack should be treated as quickly as possible. It is most important for a patient suffering an acute attack to be given extra oxygen. Rarely, it may be necessary to use a mechanical ventilator to help the patient breathe. A beta-receptor agonist is inhaled repeatedly or continuously. If the patient does not respond promptly and completely, a steroid is given. A course of steroid therapy, given after the attack is over, will make a recurrence less likely.
Maintaining control
Long-term asthma treatment is based on inhaling a beta-receptor agonist using a special inhaler that meters the dose. Patients must be instructed in proper use of an inhaler to be sure that it will deliver the right amount of drug. Once asthma has been controlled for several weeks or months, it is worth trying to cut down on drug treatment, but this must be done gradually. The last drug added should be the first to be reduced. Patients should be seen every one to six months, depending on the frequency of attacks.
Starting treatment at home, rather than in a hospital, makes for minimal delay and helps the patient to gain a sense of control over the disease. All patients should be taught how to monitor their symptoms so that they will know when an attack is starting, and those with moderate or severe asthma should know how to use a flow meter. They should also have a written action plan to follow if symptoms suddenly become worse, including how to adjust their medication and when to seek medical help. A 2004 report said that a review of medical studies revealed that patients with self-management written action plans had fewer hospitalizations, fewer emergency department visits, and improved lung function. They also had a 70% lower mortality rate. If more intense treatment is necessary, it should be continued for several days. Over-the-counter should be avoided. When deciding whether a patient should be hospitalized, the past history of acute attacks, severity of symptoms, current medication, and whether good support is available at home all must be taken into account.
Referral to an asthma specialist should be considered if:
* there has been a life-threatening asthma attack or severe, persistent asthma
* treatment for three to six months has not met its goals
* some other condition, such as nasal polyps or chronic lung disease, is complicating asthma
* special tests, such as allergy skin testing or an allergen challenge, are needed
* intensive steroid therapy has been necessary
Special populations
INFANTS AND YOUNG CHILDREN
It is especially important to closely watch the course of asthma in young patients. Treatment is cut down when possible and if there is no clear improvement, some other treatment should be tried. If a viral infection leads to severe asthmatic symptoms, steroids may help. The health care provider should write out an asthma treatment plan for the childs school. Asthmatic children often need medication at school to control acute symptoms or to prevent exercise-induced attacks. Proper management will usually allow a child to take part in play activities. Only as a last resort should activities be limited.
THE ELDERLY
Older persons often have other types of obstructive lung disease, such as chronic bronchitis or emphysema. This makes it important to know to what extent the symptoms are caused by asthma. Giving steroids for two to three weeks can help determine this. Side-effects from beta-receptor agonist drugs (including a speeding heart and tremor) may be more common in older patients. These patients may benefit from receiving an anti-cholinergic drug, along with the beta-receptor agonist. If theophylline is given, the dose should be limited, as older patients are less able to clear this drug from their blood. Steroids should be avoided, as they often make elderly patients confused and agitated. Steroids may also further weaken the bones.
Prognosis
Most patients with asthma respond well when the best drug or combination of drugs is found, and they are able to lead relatively normal lives. More than one-half of affected children stop having attacks by the time they reach 21 years of age. Many others have less frequent and less severe attacks as they grow older. Urgent measures to control asthma attacks and ongoing treatment to prevent attacks are equally important. A small minority of patients will have progressively more trouble breathing and run a risk of going into respiratory failure, for which they must receive intensive treatment.
Prevention
Minimizing exposure to allergens
There are a number of ways to cut down exposure to the common allergens and irritants that provoke asthmatic attacks, or to avoid them altogether:
* If the patient is sensitive to a family pet, removing the animal or at least keeping it out of the bedroom (with the bedroom door closed), as well as keeping the pet away from carpets and upholstered furniture and Removing hair and feathers.
* To reduce exposure to house dust mites, removing wall-to-wall carpeting, keeping humidity down, and using special pillows and mattress covers. Cutting down on stuffed toys, and washing them each week in hot water.
* If cockroach allergen is causing asthma attacks, killing the roaches (using poison, traps, or boric acid rather than chemicals). Taking care not to leave food or garbage exposed.
* Keeping indoor air clean by vacuuming carpets once or twice a week (with the patient absent), avoiding using humidifiers. Using air conditioning during warm weather (so that the windows can be closed).
* Avoiding exposure to tobacco smoke.
* Not exercising outside when air pollution levels are high.
* When asthma is related to exposure at work, taking all precautions, including wearing a mask and, if necessary, arranging to work in a safer area.
More than 80% of people with asthma have rhinitis and recent research emphasizes that treating rhinitis helps benefit ashtma. Prescription nasal steroids and other methods to control rhinitis (in addition to avoiding known allergens) can help prevent asthma attacks. It is also important for patients to keep open communication with physicians to ensure that the correcnt amount of medication is being taken.

A novel drug that enables the production of normal proteins from mutated DNA might one day help people with a variety of genetic diseases. The drug has shown promise as a treatment for cystic fibrosis and muscular dystrophy, and it is now being tested in large, international clinical trials.
Most drugs alter the activity of proteins after theyre manufactured, but the new drug intervenes in the cellular machinery that makes the proteins in the first place. Consequently, it could be effective against diseases where completely different proteins go awry. s a great breakthrough, says Robert Singer, a biologist at the Albert Einstein Medical School, in New York, who is not involved with the company that produces the drug.
Severe genetic disorders, such as muscular dystrophy, result from mutations in genes that code for vital proteins. In some cases, the mutation is a misplaced genetic stop sign, a sequence that tells the cellular machinery to halt production before the protein is complete. The result can be a truncated, ineffective version of the protein, or none at all. The new drug, being developed by PTC Therapeutics, a startup in South Plainfield, NJ, allows the cellular machinery to essentially skip over these aberrant stop signs and produce normal molecules.
While severe genetic diseases are individually rare, mutations that prematurely truncate protein production are found in many of themincluding spinal muscular atrophy, hemophilia, and retinitis pigmentosa. Since the drug treats the underlying gene-expression problem, it is applicable to a few thousand diseases, says Allan Jacobson, chair of the department of Molecular Genetics and Microbiology at the University of Massachusetts Medical School, in Worcester, and a PTC cofounder.

The music's pretty; I have a string quartet version that didn't make it onto the soundtrack (from a promo CD) that's lovely. It doesn't need Celine Dion or lyrics to work. It sounds better without them.
My Favorite Nominee: Ray of Light, Madonna. I don't have much to say about it, but it works.
My Favorite Single That Year: Flagpole Sitta, Harvey Danger. That song just makes me feel awesome. I love it.

Winner: Sunny Came Home, Shawn Colvin. Music from the nineties has an overwhelming percentage of suck, more than any decade. It's like America went through menopause and could only listen to this kind of sappy pap. I hate this thing, and thanks to the Crap and Crap Lite stations being played where I worked constantly, I heard it way too many fucking times.
My Favorite Nominee: MMMBop, Hanson. It's a default choice; it's the one I think is okay whereas I despise all the others (especially "Where Have All the Cowboys Gone" by Paul Cole, which should be classified a form of abuse).

Winner: Change the World, Eric Clapton. I'm not much of a Clapton fan, really. I did like this song, although it's association with the awful Scientology-promoting John Travolta film Phenomenon counts against it a tad. It's not earth-shaking, but it's a solid, not-unpleasant song.

My Favorite Nominee: Losing My Religion, R.E.M. It was overplayed, but if you listen to it now, it sounds almost fresh again. It really is just a good song.

Winner: Another Day in Paradise, Phil Collins. Preachy, annoying, and not even the best song from that Phil Collins album. (Actually, I just checked and sadly, it is. I despise "Something Happened on the Way to Heaven," and "I Wish It Would Rain" just sounds like a rip-off of "Wish You Were Here" with Clapton on guitar.)
My Favorite Nominee: Nothing Compares 2 U, Sinead O'Connor. Of the sappy, preachy, sad sack songs that were nominated this year, this is the one that's actually a good song. (Also, "U Can't Touch This" was nominated this year, but come on, man.)

Winner: Wind Beneath My Wings, Bette Midler. I hate this song, and my dad pissed me off by playing it at his wedding reception for his mother, which I specifically told him not to do because it was such a fucking cliche. He said he wouldn't; he did. Wow, my grandma must have been one of 10 million special woman so uniquely honored that year. It's the equivalent of buying your dad a tie on Father's Day.
My Favorite Nominee: The End of the Innocence, Don Henley. Chance is right on when he calls it deceptively angry. It adds some world-weariness on top of that, too. Beautiful song.

Heart Will Go On, Celine Dion. This was very easy to get sick of as well; cloying and over-the-top and insisting on its own epic greatness. Blurg. The music's pretty; I have a string quartet version that didn't make it onto the soundtrack (from a promo CD) that's lovely. It doesn't need Celine Dion or lyrics to work. It sounds better without them.
My Favorite Nominee: Ray of Light, Madonna. I don't have much to say about it, but it works.

Winner: Sunny Came Home, Shawn Colvin. Music from the nineties has an overwhelming percentage of suck, more than any decade. It's like America went through menopause and could only listen to this kind of sappy pap. I hate this thing, and thanks to the Crap and Crap Lite stations being played where I worked constantly, I heard it way too many fucking times.
My Favorite Nominee: MMMBop, Hanson. It's a default choice; it's the one I think is okay whereas I despise all the others (especially "Where Have All the Cowboys Gone" by Paul Cole, which should be classified a form of abuse).
My Favorite Single That Year: The End Is the Beginning Is the End, Smashing Pumpkins. I love that they used it in the Watchmen trailer.

Winner: Change the World, Eric Clapton. I'm not much of a Clapton fan, really. I did like this song, although it's association with the awful Scientology-promoting John Travolta film Phenomenon counts against it a tad. It's not earth-shaking, but it's a solid, not-unpleasant song.

Winner: Unforgettable, Natalie Cole. The fact that the Grammys honored that hacky, schlocky, sympathy-begging, cloying bit of grave-robbing Natalie Cole did to cash in on honor her father is as sad as it is unsurprising.
My Favorite Nominee: Losing My Religion, R.E.M. It was overplayed, but if you listen to it now, it sounds almost fresh again. It really is just a good song.

Winner: Another Day in Paradise, Phil Collins. Preachy, annoying, and not even the best song from that Phil Collins album. (Actually, I just checked and sadly, it is. I despise "Something Happened on the Way to Heaven," and "I Wish It Would Rain" just sounds like a rip-off of "Wish You Were Here" with Clapton on guitar.)
My Favorite Nominee: Nothing Compares 2 U, Sinead O'Connor. Of the sappy, preachy, sad sack songs that were nominated this year, this is the one that's actually a good song. (Also, "U Can't Touch This" was nominated this year, but come on, man.)
My Favorite Single This Year: Enjoy the Silence, Depeche Mode. Now there's a love song.

Winner: Wind Beneath My Wings, Bette Midler. I hate this song, and my dad pissed me off by playing it at his wedding reception for his mother, which I specifically told him not to do because it was such a fucking cliche. He said he wouldn't; he did. Wow, my grandma must have been one of 10 million special woman so uniquely honored that year. It's the equivalent of buying your dad a tie on Father's Day.
My Favorite Nominee: The End of the Innocence, Don Henley. Chance is right on when he calls it deceptively angry. It adds some world-weariness on top of that, too. Beautiful song.
My Favorite Single That Year: A Little Respect, Erasure. I usually come out on the side of pop, I guess. Although besides the catchiness, I think the lyrics are beautiful. One of my all time favorite lyrics comes from this song: "What religion or reason could drive a man to forsake his lover?"

Winner: Don't Worry, Be Happy, Bobby McFerrin. I always liked this song, but it sure wasn't the best of the year. I think part of it was the novelty of McFerrin doing the whole thing a cappella. Which is admittedly neat.

She is old, body withered with osteoporosis and arthritis, mind withered with Alzheimer's and dementia. A bloodstain spreads out across the carpet from where she fell. She has a cut and goose-egg, swelling over one eye. She has neck pain, when I gently palpate it.

She is not happy.

We put her on the backboard, reluctantly, but recognizing it as the right move.

She does not agree.

"Put me back! You don't know what you're doing! Put me back, you little sh*ts!"

We trade glances with each other, with the staff. One of them shrugs, half-apologetically. She's always like this, they say. We nod, and load her up. My partner smirks, first from the rear door, and then from the rear-view mirror as we drive in, and I am bombarded with confused, frustrated, furious insults.

"You're a bunch of brainless farts! You don't know what you're doing! Brainless farts!"

"Ma'am, you fell, we're taking you to the hospital, you need to see the doct--"

"Shut up!"

"Ma'am, we're taking care of you, th--"

"Shut UP!"

"Ma'am, I--"

"Shut up shutupshutupSHUTUP!"

He sits on a bar stool, holding his belly. We carefully help him step to the stretcher. Flickering LED lights from the eight or ten police cruisers outside illuminate the club in sporadic bursts of light. He doesn't seem to be bleeding too much.

Quickly we load him up. My partner grabs a set of vitals while I put him in the trauma system. We're less than fifteen blocks from the Level I; a ten minute walk, let alone a drive. My partner reads off the pressure and heart rate and then bails out for the driver's seat. I wipe the man's belly off with some gauze, and see that there's only a single stab wound. Doesn't even look that bad. His blood pressure and, more importantly, heart rate are fine.

As we pull out from the club, I quickly pull down supplies to put an IV in. This close, I can't do the two I prefer, but I can at least get one.

We've been driving for thirty seconds when I realize I can still hear the strobes clicking. I shout to my partner.

"Code one, code one, shut it down! We're fine going code one."

"Right," he says, and I hear the lights go off.

"Ain't got no damn time anyways," I mutter to myself, taping the IV down ninety seconds later as we turn into the hospital driveway.

Ten minutes after that -- and maybe half an hour after the call came in -- he's on the operating room table. Despite my earlier frustration, I smile to myself. That, I think, is good trauma care, right there.

She is younger, and the confusion comes from the case of tallboy beers, not dementia, but just the same she too does not want the backboard, or our help. A giant scalp laceration, easily a foot long, winks at me as we tape her head down.

In the ambulance, she calms down. I explain what's happening, repeatedly, kindly, and gently coax the facts of what happened out of her. As we drive in to the hospital, she goes from argumentative to cheerful, albeit confused.

"'zis'sh an airplane?"

"Uh, no, ma'am, it's an ambulance."

"... sounds like we're in'na air."

"We're on the freeway, ma'am."

She chuckles. "No! You're full of it. Stop messin' wi' me."

I smile, myself. "I would never mess with you." I stand up, so I can meet her backboarded gaze.

"Whoa! How'dsh you do that? 'm I standing on m'head?"

"Uh ... no, ma'am, you're flat, and I'm standing next to you."

She can't seem to figure out how the stretcher and ambulance configuration works, and chortles the whole rest of the way that I'm messing with her, and it must be some kind of a trick. But she's jovial enough. I explain three more times that she needs stitches, and to make sure her neck isn't hurt.

"Whatever," she says, patting my knee. "I'm fine, I know it."

When we get to the hospital, on the way in, she holds up a hand. "Thanks," she says. "You've been really nice to me."

I shake her hand. "You're very welcome, ma'am. I just hope you get well."

She twists under the tape and collar, trying to look at me. "You do good work, sonny." The hospital doors open smoothly, and she looks up at the bright flourescent lights and white walls.

"Oh hell, whatsh this?"

He is old, but not that old.

He is naked, lying on the living room floor.

He is dead, most assuredly, even though my partner is still doing CPR, a fireman is still bagging, and another fireman is watching the monitor and pushing drugs.

We've been here for half an hour, through a few rhythm changes, none responding especially well to treatment. Vasopressin kicked the asystole to fib, but shocks kicked the fib to PEA, and there it's stayed, complexes widening and bradying, despite epi, fluid, atropine, and good CPR. His head and neck have begun to purple and mottle.

Finally the firemedic running the code nods to himself, and starts reviewing the drugs we've given. "We're at 35 minutes. Our end-tidal is--" he glances at the Phillips monitor and winces, "--8. Anyone have any other ideas?"

A roomful of silence. "Okay. Four-twenty-two, then."

I stand, walk outside, take a breath of cold night air. Walk down to the idling ambulance. Grab a white hospital blanket and a couple of red plastic biohazard bags. I pass the bio bags to my partner and spread the blanket over the dead man.

Five minutes later we're outside again, my partner with a copy of the paperwork. The firemen agreed to wait for the M.E. and cops -- only fair, as they arrived first.

I reach up, key the mic riding my shoulder. "Medic 38 in service.

John McCains speech was particularly humbling, instructive and inspiring. If in Africa, incumbents would accept defeat and would graciously depart from the seat of power, this would be a different continent, and indeed Zimbabwe would be a different place.
For those of us who are still in the trenches, fighting for change and democracy across the entire African continent, this is our victory. One which for now we will savor and celebrate.
